Complex Nursing care in your own Home
If you are assessed as needing more clinically focussed care, we can provide Registered Nurses to plan and implement a package of care to address your specific requirements.
Nurses can work autonomously, and this allows your care plan to be continuously evaluated and updated when appropriate to meet your needs.
Your care package will be truly holistic and will be developed in partnership with you, your family and the other health and social care professionals involved in your care. When required we work closely with acute hospitals and other in-patient settings to ensure your discharge and transition to home care is seamless.
Holistic care places you at the centre; your physical, psychological and social needs will be met whilst ensuring you maintain the quality of life you are accustomed to at what can be a challenging time.
Registered nurses are trained professionals, registered with the Nursing and Midwifery Council (NMC). Our nurses are selected based on their specific skills and experience related to the services we offer; they will use their skills to make the clinical decisions required as your care needs change.
